A note from our Medical Director:

Adam M. Carpentieri, D.O, Medical Director

Welcome to White Oaks!
Located beside an idyllic pond in Woodbury New York, the family owned and operated White Oaks Rehabilitation and Nursing Center offers the complete package of geriatric care. The tranquil location sets the stage for the compassionate nursing care rendered for both long term residents and short term physical rehabilitation patients alike.
As a medical director of several nursing and rehabilitation facilities in both Nassau and Suffolk counties, mine is a unique perspective. White Oaks prides itself on delivering quality, comprehensive care for each patient. The nursing staff is attentive, the physical therapy staff is skilled, and the social workers are caring. The facility also provides the valuable and unique benefit of having multiple medical and surgical sub specialists on staff. This allows patients to be treated for a plethora of medical maladies without the need for transport to off site locations. We offer board certified physicians in internal medicine, family medicine, cardiology, pulmonology, palliative care, neurology, dermatology, gastroenterology, psychiatry, general surgery, orthopedic surgery, urology, podiatry and wound care. They are available to our patients 24 hours per day, seven days per week.

These facility wide values have enabled us to forge strong relationships with the both the patients and their families. This is evidenced by the multitude of referrals generated by former patients, as well as patients who return for rehab after subsequent surgical procedures. Come experience the difference White Oaks can make for you or your family member.
