Our certified social services staff plays an integral role to support residents and families during their transition to the facility, as well as coordinating a safe discharge back to the community.

As part of the interdisciplinary team, our Social Workers assess residents on a continuous basis to facilitate the best experience and quality of life for them and their families.

Discharge planning for short-term residents begins on the day of admission, and our Social Work staff coordinates home care services and medical supply providers for the convenience and safety of our residents.
